ACT Home Care is a New York–based home care organization dedicated to delivering compassionate, person-centered support.
As an approved Consumer Directed Personal Assistance Program (CDPAP) facilitator in partnership with Public Partnerships LLC (PPL), the statewide fiscal intermediary for CDPAP, ACT Home Care supports Medicaid-eligible consumers across all 62 New York State counties, helping them maintain independence, dignity, and control over their care while remaining safely at home and in their communities.
ACT Home Care guides consumers and their designated personal assistants through every stage of the CDPAP process, including enrollment, caregiver onboarding, and ongoing education, in accordance with New York State Department of Health and PPL requirements. In addition to traditional long-term care cases, ACT Home Care can service eligible populations including children, individuals with Traumatic Brain Injury (TBI), Nursing Home Transition and Diversion (NHTD) participants, and individuals served through the Office for People With Developmental Disabilities (OPWDD).
Rooted in Advocacy, Compassion, and Trust, ACT Home Care places the consumer at the center of all decisions and works collaboratively with caregivers, and community partners to deliver reliable, compliant, and responsive support.
Through its partnership with PPL, ACT Home Care is proud to serve as a trusted CDPAP facilitator, enabling individuals to receive care from the people they know and trust.

Transition and Registration Information

EVV and Payroll
There are two EVV options for consumers and PAs:
Option 1: Mobile App
Download the Time4Care mobile application on your smartphone. The consumer and PA can each download the app separately on their phones, or the consumer can use the PA’s phone to review and approve shifts. If you have an Android device, use this link to download from Google Play:

Option 2: Using Telephony
Use a landline or consumer’s cell phone to call in and out through the EVV phone line (the PA has to call from whichever phone number the consumer used to register with PPL) at 833-278-3959 and follow the steps below:
- Select Option 1 to indicate that you are the caregiver
- Enter your eight-digit date of birth
- Enter the last four digits of your Social Security Number
- Select your consumer from the automated list
- Follow the prompts to clock in or out
Consumer approval of PA hours worked
In order for PAs worked hours to be processed, the consumer or DR must approve all hours worked each week by Sunday 11:59 PM for it to be processed in PPL’s weekly payroll. There are 3 ways for consumers/DRs to view and approve hours worked: Time4Care, PPL@Home or via Telephony. All time entries, including paper timesheets, are due for submission to PPL on each Saturday at 11:59 p.m. ET, Consumer approval is due Sunday at 12 p.m. ET. Please only submit one timesheet to avoid processing delays. Paper timesheets must be submitted using PPL’s official timesheet for New York CDPAP. Timesheets from other sources or programs will not be accepted.
Timesheets
There are 2 ways for PAs to log missed time: Time4Care, PPL@Home. Please note logging missed time is not EVV compliant. Paper timesheets aren't method for logging missed time. It requires an exception that is only granted in special circumstances.
Logging missed time via the Time4Care app
To log time in Time4Care for hours that you were not able to use the app, i.e. Sabbath, please log into Time4Care and tap add past shift from the dashboard.
Logging missed time via Telephony
To log missed time via phone, please dial 833-278-3959 and follow the prompts for logging missed time
Logging missed time via PPL@Home
To log missed time via PPL@Home, log in with your username and password and navigate to the timesheet tab.
PAs who are unable to log time via Time4Care, Telephony or PPL@Home, may submit missed time via paper timesheets.

No Smartphone, Tablet, or Internet?
Medicaid members may qualify for a free smartphone through a federal program.
